ADULTS

Sedation Options
*Doses are for initiation of treatment. Titrate up PRN.
Fentanyl: 25 – 100 mcg
Midazolam: 1 mg given over 2 min (up to 5 mg)
Ketamine: 6-13 mg/kg IM injection

Reversal Agents
Naloxone: 0.1 – 0.2 mg IV (or 0.4 mg IM)
Repeat every 2-3 minutes PRN
Use this dosing for reversal of postoperative respiratory depression
Flumazenil: 0.1 – 0.2 mg IV over 30 sec
Repeat every 2-3 minutes PRN up to Max dose 5 mg

Preop Antibiotic Prophylaxis Options
Cefazolin: 1-2 g IV
Ceftriaxone: 1-2 g IV
Vancomycin: 15 mg/kg up to 1750 mg preop
Gentamicin: 1.5 mg/kg
Ampicillin/Sulbactam: 3 g IV
Ciprofloxacin: 400 mg IV
Clindamycin: 600 mg IV
Metronidazole: 500 mg IV

Analgesic Options
*Doses are for initiation of treatment. Titrate up PRN.
Hydromorphone PO: 2-4 mg q 4-6 hours
Hydromorphone IV: 0.2-1 mg q 2-3 hours
Hydromorphone PCA: demand dose 0.1-0.2 mg; lockout 5-10 min
Ketorolac PO: 10 mg q 6 hours
Ketorolac IV: 30 mg q 6 hours

Anti-hypertensives
*Doses are for initiation of treatment. Titrate up PRN.
Labetalol: 20 mg IV, then 40-80 mg IV q 10 min PRN (up to 300 mg)
Metoprolol: 50 mg BID titrate up PRN (up to 450 mg/day)
Hydralazine: 20-40 mg IV, repeat PRN

GI Agents
Ondansetron: 4 mg IV or 16 mg PO
Glucagon: 1 mg IM

Other
Sotradecol: 1 mL of 1% solution into each varices, up to 10 mL max
